CHAPTER 1: DECIDE

Most of us have long forgotten some of the passions of our youth or early adult years. This worksheet helps you rediscover those passions. This can be considered a subcategory of your brainstorming, but it is important enough to deserve its own worksheet.
CHAPTER 2: INVESTIGATE
This worksheet, also used in Chapter 1, guides you through the process of a brainstorming session and provides guidelines and prompts to stimulate discussion.

CHAPTER 3: SORT
The Sort Worksheet serves as a reference when sorting material. It helps you walk each piece of data through your belief system, reticular activating system, credibility analysis, and the organizational process. It also summarizes the tagging scheme and will help you grade your materials consistently.
CHAPTER 4: CONCEIVE
This detailed worksheet will walk you through the six steps presented in this chapter and help you develop a plan for effective change.
CHAPTER 5: IMPLEMENT
Use the DISCIPLINE STRATEGY Daily Journal page (also used in Chapter 1) to capture the specifics of your daily grind. This should include reminders of your new end-of-day habits like recording your bite-sized victories, recording thoughts in your gratitude journal, and any other daily celebrations. The Daily Journal is designed with room for you to customize it for your specific purposes.
CHAPTER 6: PERSEVERE
As mentioned earlier in this chapter, the Obstacles and Perseverance Worksheet is designed to help you quantify the obstacle or significant challenge you’re facing and prepare a plan to persevere until you solve the issue or come up with a suitable workaround. CHAPTER 7: LOOP
Asking friends and colleagues for honest feedback can be a tricky proposition. Without clear guidelines, well-meaning respondents could simply tell us what they think we want to hear, give us a broad overview with no specifics, or even try to talk us out of our goals. Avoid these common feedback pitfalls by walking your feedback source through a clear, actionable conversation using the questions and categories on this form.
We are too often our own worst critics. We generally think either too highly of ourselves or we only see our failures; neither option provides valuable insights. The Self-Assessment Worksheet, however, guides you in an honest look at yourself and equips you to list your strengths, weaknesses, opportunities, and threats in an effective, unbiased way.

This worksheet will help you identify and plan for the realignment of your priorities during this critical phase of the DISCIPLINE STRATEGY.
CHAPTER 9: NOTICE
The Notice Worksheet helps you to commit to celebration, assess alignment of your current goal with your internal desires, begin the engineering process for your next goal, document and lock in your areas of personal growth, and inspire you to level up. Use this worksheet as you approach your goal or immediately after you have attained it. This will serve as a foundation for starting the DISCIPLINE STRATEGY with your next goal.
CHAPTER 10: ENJOY
The Guide to Happiness and Well-being is a pack of worksheets designed to walk you through each piece of the PERMA puzzle. In these forms, you’ll work through exercises to help you build positive emotion, engagement, relationships, meaning, and accomplishments.
